Greene County Public Health is conducting a Community Health Needs Assessment now through April 15.
The assessment is critical in identifying the health needs of Greene County communities and residents. The survey is available in English [click here for English survey] and in Spanish [Haga clic aquí para ver la encuesta en español]. QR codes are also provided.
The Community Health Assessment and Improvement planning (CHA CHIP) has more than a 30-year history in Iowa and represents vital local action to promote a protect the health of Iowans.
Every five years, Greene County Public Health assures a community-wide process takes place to identify the most important factors affecting the health of residents and to plan strategies to build on strengths and work on gaps.
Greene County Public Health encourages each resident to provide feedback about what they feel are health concerns via use of a simple survey. Based on the results of the survey, Greene County Public Health researches data related to the identified issues, and then facilitates the collaboration of community partners to assist with developing a Health Improvement Plan (HIP) to attain the common goal of a healthier Greene County. The HIP is updated annually to ensure that progress continues.
